Ongoing Support & Care
Letters, Forms & Accommodations
New patients must first complete a psychiatric evaluation. Fees cover professional review and completion, not a guaranteed diagnosis, letter, or approval.
-
Florida-compliant housing documentation provided after a genuine clinical assessment confirms a disability-related need and a letter is clinically appropriate.
Not an online certificate or quick registration. Approval isn't guaranteed.
-
Documentation outlining psychiatric symptoms, treatment history, and functional limitations to support leave or intermittent leave when clinically warranted.
Employer approval and benefit eligibility are determined by your employer or plan, not by this office. -
Clinically grounded requests for reasonable accommodations — such as adjusted scheduling or a reduced-distraction workspace — based on your needs.
The specific accommodations granted depend on clinical necessity and your employer's internal process. -
Clinical documentation to support appropriate academic or testing accommodations for established adult patients.
Some institutions may require separate neuropsychological or psychoeducational testing. -
An up-to-date clinical evaluation of psychiatric stability, including any recommended restrictions, when returning to work or school.
Fitness-for-duty, public-safety, court, and forensic evaluations may require an independent specialist. -
A letter confirming current treatment or prescribed medication, often needed for travel, school, employment, or coordination with another provider.
Only the minimum necessary information is disclosed, and only with your authorization.

Your First Visit
Starting psychiatric care can feel unfamiliar, especially if it’s your first time. My goal is to make your first visit comfortable, thorough, and straightforward—so you know what to expect from the moment we begin.
01
You talk. I listen.
We’ll begin with what brought you in and what you’d like help with. You’ll have space to speak openly and in your own words, without feeling rushed. Visits are available in English or Spanish, based on your preference.
02
We look at the bigger picture.
We’ll explore your health history, sleep, work, relationships, previous treatment, and other factors that may be affecting how you feel. Taking time to understand the full picture helps create care that is individualized.
03
Understanding your options.
I’ll share my clinical perspective in clear, easy-to-understand language. Together, we’ll discuss what may be contributing to your symptoms and explore treatment options, including medication when appropriate.
04
A clear path forward.
We’ll end your visit with a plan that feels clear and manageable. If medication is appropriate, prescriptions can be sent to your pharmacy, and we’ll schedule your next appointment so you know exactly what comes next.
Who We Are
We are committed to care that is both clinically sound and genuinely compassionate — taking the time to understand what you're experiencing, answer your questions, and build an individualized plan that fits your needs and your life.
